These documents are essential for verifying your identity, confirming your eligibility, and helping us provide you with the best possible care.
To provide you with the best care possible, we need to understand your medical background and any prior treatments you’ve received. Please bring the following items to your first appointment:
We require records that document your diagnosis and any treatments you’ve received. Please bring relevant medical records documenting your diagnosis and treatments, along with authorization for release. Include clinical notes from your primary or referring physician, diagnostic imaging like X-rays, CT, MRI, or PET scans with reports, pathology, lab, and surgical reports, and any treatment summaries if available.
To coordinate your care, your PCCSS specialist may need to connect with your primary care physician or referring doctor. This enables us to share information and collaboratively determine the best approach for your treatment plan. With your permission, we may send updates, test results, and reports to your other healthcare providers.
Please bring your insurance card and a valid photo ID. If your insurance provider requires a referral, ensure it has been sent to us by your referring doctor. We will verify your coverage, discuss any potential out-of-pocket costs, and file insurance claims on your behalf. If we cannot confirm your referral or insurance coverage before your appointment, it may be rescheduled, or you may be financially responsible for the visit.
We want to ensure you have all the information you need. Bring a list of questions to discuss with your doctor, and we’ll take the time to address each one. We’ll explain your diagnosis, review treatment options, and outline the next steps to move forward.
You may be required to complete several patient forms at your first visit. To save time, you can print and complete these forms in advance and bring them with you.
Please bring a detailed list of all medications you’re currently taking, including dosage and frequency, or bring the original packaging with you. If available, you may also bring a completed Medication and Allergy List.
